Before completing this application form, you should have read the Power of Her 2027 Recognition Awards Guidelines.

Incomplete nominations and/or nominations submitted after the closing date will not be considered.

This application form is designed for us to best understand the nominee's contribution to women and girls in sport throughout 2026.

It is important that you complete these questions with as much evidence as possible and provide details relevant to the selection criteria. Dot points are acceptable within the application and concise answers with supporting documentation attached are encouraged.

Before proceeding, please confirm the following:

• You have read and understood the Award Guidelines. 
• You are able to demonstrate alignment between the nominee and the relevant award criteria.
• Your organisation/nominated individual is located in South Australia.
• Your nomination is endorsed by a State Sporting or Recreation Organisation, Association or Club, and you will attach a letter of endorsement where required.
